1. If you are currently experiencing or have experienced in the last three weeks, signs of acute respiratory illness such as coughing, fever, and shortness of breath.
2. If you have had close contact with an individual diagnosed with COVID-19, please contact our office prior to your appointment.
· If you have had a fever associated with a dental diagnosis (e.g., pulpal and periapical dental pain and intraoral swelling is present), but no other signs/symptoms of COVID-19 (e.g., fever, sore throat, cough, difficulty breathing), you can be seen in dental settings.
· CDC recommends that only asymptomatic patients, patients who have tested negative for COVID-19 infection, or recovered patients (after three days since resolution of signs and symptoms) be seen in dental settings.
Please limit extra companions on your trip to the office to help reduce the number of people in the reception area. We understand this may not be possible, however we appreciate you doing what is reasonable to keep crowding down for the time being. Please fill out the COVID-19 Patient Screening form.
